At C4M, we believe that you shouldn’t have to navigate the healthcare system alone. Our comprehensive case management program allows us to become your healthcare ally. Our staff develops a consistent caring relationship with you and your family. We assist you to define your needs as well as active and needed providers; we then work to connect you with the services that help you establish and maintain your healthcare goals.

Comprehensive Care Management


No one should have to manage a chronic illness or disabling medical condition on their own. You and your family need a dedicated partner to help you make the most of your healthcare system. We do our part by providing comprehensive case management and workforce finding services. Our team evaluates your informal and formal support systems and works to find quality resources to fill the gaps.


  • We assist you in making decisions based on the care choices you prefer.
  • We help you access high quality, cost-effective options you might not know about.
  • We work with local service and support organizations.
  • We assist you in understanding your insurance coverages navigating prior authorization and access to prescription benefits. 
  • We assist you in creating emergency and crisis plans to ensure safety and support even at the most difficult times.

LTSS Workforce Finding Services


If you and your case management determine that there are gaps in your home health providers, our LTSS Navigators will join the team and assist you in filling those gaps. Our LTSS navigators;


  • Work with you to define your in home care needs profile, including type of specialty and specific skills, preferred schedule, language and cultural needs, etc.
  • Recruit and screen potential ih home providers.
  • Assist you in selecting and training in home providers.
  • Assist you in developing a schedule for in home providers, including a back-up plan to cover in the event of call outs.
