Person-Centered Case Management and LTSS Workforce Finding Services to Meet Complex Care Needs


Comprehensive Care Coordination/Case Management (C4M) is a compassionate team providing person-centered, culturally competent and high-quality case management and long-term services and supports (LTSS) workforce finding services for eligible C4M enrollees.


Our agency has over 75 years of experience providing comprehensive care for individuals and families with complex needs. We use our deep working knowledge of community resources to assist enrollees with complex care needs identify and access needed supports and services. Our team coordinates home and community resources and care across a broad spectrum of health and social services.


  • The Case Managers who work directly with you are the core of our team. Care managers are selected for their knowledge of community resources and experience coordinating services for individuals with complex care needs. Case managers know the communities where they work, and many speak the languages of the people they serve.


  • C4M includes a Care Resource Support team which includes experts in pharmacy, education, community resources (such as transportation, housing, food), home ADA accommodations, durable medical equipment and behavioral health. Team members may provide guidance to the case manager or work directly with you, depending on your needs and preferences.


  • C4M includes Long Term Services and Supports workforce finding supports as well, because we understand finding qualified, reliable home health staff can be challenging. Our team supports you in defining what kind of help you need, assists you in finding a good fit provider, training that provider, and establishing a backup plan for when the providers are not available (planned and unplanned time off, or leaving the position).


  • Our case management team has diverse experience with children, families, and adults, and the services critical to their health and survival.


  • We focus on person-centered case management because we believe enrollees are the experts on their needs.


  • We work closely with each member and take their life situation into account so they can be as healthy as possible.


  • We have experience with crisis intervention, persons with disabilities, behavioral health, individuals with intellectual and developmental disabilities, domestic issues and other critical health and social service areas.



  • As part of your care team, we’re here to connect the dots and promote communication between the primary care team, all involved parties and you.
