JOB TITLE: Care Coordinator

PROGRAM: Intensive Care Coordination

LOCATION: Springfield

EMPLOYMENT TYPE: Full-Time

SCHEDULED HOURS: 40 hours

SCHEDULE / SHIFT: Monday – Friday, flexible between 8AM-8PM depending on appointments

BONUS: $2,500 Sign-On Bonus

RESPONSIBILITIES:

Qualified professionals needed to join an innovative team of care coordinators and family partners using a Wraparound approach to provide intensive care coordination for youth with serious emotional disturbance and their families. The successful candidate will conduct strengths, needs and culture discoveries, facilitate care planning teams, craft individualized care plans and safety plans, and work with families and teams to implement care planning strategies. Bilingual/Bicultural, Spanish-speaking candidates encouraged to apply.

QUALIFICATIONS:

  •  Applicants must be experienced working with children and families, and have a Master’s or Bachelor’s degree in the mental health field or an Associate’s degree or HS Diploma plus five years experience working with the target population.
  • Must have a valid driver’s license and an automobile.
  • Must be CANS-MA certified.
